چکیده

Key points † Optimizing tracheal sealing is of clinical interest: fluid leakage past the tube cuff plays an important role in the pathogenesis of VAP. † Tapered shaped tube cuff offering a sealing zone with reduced folds and channels is a new strategy to minimize fluid leakage past the tracheal tube cuff. † Tracheal tube cuffs made from PVC show much more and faster fluid leakage than cuffs made from PU in a simulated artificial model. † Tracheal sealing with tapered shape PU tube cuff is as efficient as the conventional cylindrical PU tube cuff in smaller tracheal diameters and more efficient at higher tracheal diameter with regard to tracheal sealing against fluid leakage. Background. This study compared the fluid leakage in the new ‘tapered’ shaped against the classic ‘cylindrical’ shaped tracheal tube cuffs when placed in different sized tracheas.

Rapid pressure compensation by automated cuff pressure controllers worsens sealing in tracheal tubes.

BACKGROUND Cyclic redistribution of air within the cuff during respiratory pressure changes creates a self-sealing mechanism which allows tracheal sealing, despite tracheal airway pressure being above baseline cuff inflation pressure. The aim of the present study was to investigate the effect of continuous automated cuff pressure regulation on tracheal sealing during cyclic respiratory pressure...

[Effects of tracheal tube cuff limit pressure (25 cmH2O) and "seal" pressure on tracheal mucosa of dogs.].

BACKGROUND AND OBJECTIVES Injuries of tracheal mucosa in contact with tracheal tube cuff is a function of cuff pressure and exposure time. This study aimed at analyzing injuries of tracheal mucosa in contact with tracheal tube cuff inflated to reach "seal" pressure or limit 25 cmH2O pressure, below critical 30 cmH2O, to prevent tracheal damage. METHODS This study involved 16 dogs submitted to...

Effect of tracheal suctioning on aspiration past the tracheal tube cuff in mechanically ventilated patients

UNLABELLED BACKGROUND This clinical study evaluated the effect of a suctioning maneuver on aspiration past the cuff during mechanical ventilation. METHODS Patients intubated for less than 48 hours with a PVC-cuffed tracheal tube, under mechanical ventilation with a PEEP ≥5 cm H2O and under continuous sedation, were included in the study. At baseline the cuff pressure was set at 30 cm H2O. ...

Evaluation of the optimal cuff volume and cuff pressure of the revised laryngeal tube “LTS-D” in surgical patients

BACKGROUND Recent case reports have indicated significant cuff overinflation when using the standard filling volume based on the manufacturer's recommendations in older models of laryngeal tubes. The aim of this study was to determine the minimum cuff pressure needed to perform standardized ventilation without leakage in the new, revised model of the laryngeal tube "LTS-D". METHODS After ethi...

Assessment of safe endotracheal tube cuff pressures in emergency care - time for change?

To the Editor: Endotracheal intubation is performed in the pre-hospital and emergency department (ED) environments by advanced life support (ALS) paramedics and emergency doctors. Cuffed endotracheal tubes (ETTs) are used in adults and more recently in children 1 to ensure that the airway is protected, and to prevent air leakage between the wall of the trachea and the ETT during positive-pressu...
